Book an Appointment

Please fill out the form to request an appointment. Once you hit Send, you will receive and email to confirm that we have received your request. If you selected your preferred doctor, your request will be sent directly to his/her personal receptionist for processing. If you selected "Any available doctor" we will do our best to book an appointment for you with the first available doctor.

You will be contacted via email or phone to confirm your booking, or to suggest an alternative time and/or date depending on openings in our doctors' booking schedules. You may also be asked to see another doctor if your preferred doctor is on vacation or not taking any new patients.
