Our clinic is part of the Big Country Primary Care Network. They provide professional staff as a part of our multidisciplinary team approach to enhance and deliver health care services. Presently we have a Family Nurse Practitioner, Registered Nurses, Wellness Coordinator and Referral Navigators working with us.
Our PCN registered nurses are responsible for patient education and follow-ups for chronic disease management. This includes but is not limited to conditions such as diabetes, hypertension, cholesterol, asthma, COPD. PCN nurses can also help with health promotion and prevention for all ages.
We have a wellness coordinator that offers classes and patient education on healthy lifestyles including weight loss, tobacco cessation and exercise options for patients with physical challenges.
The referral navigator’s role is to facilitate patient referrals to specialists, specialty clinics, and other medical investigations. If you have concerns or questions about referrals to a specialist outside of our facility our PCN Navigators can answer your questions.
Patients can self-refer to see the nurse and the wellness coordinator.
